The elbow is comprised of three articulations: radiocapitellar, ulnotrochlear, and proximal radioulnar joints. Soft- tissue injuries to the elbow are common, but most injuries to the elbow requiring emergency treatment involve either fracture or gross elbow instability. Radiographs are performed for initial evaluation and are typically adequate for excluding injuries requiring urgent intervention. CT of the elbow should be performed in periarticular fractures to assess for intra-articular extension and intra-articular fractures for preoperative assessment. CT angiography can help evaluate for arterial injury, but is rarely necessary as vascular injury is uncommon in the setting of elbow trauma. MRI is the preferred imaging modality for assessing most soft-tissue injuries, but is rarely performed in the emergency setting. Ultrasound can be used as an alternative to MRI for soft-tissue injury assessment, but has limited use in the acute setting due to challenges in positioning acutely injured patients, as well as the high level of expertise required to perform appropriate imaging assessment.
