Optimal Technique of Phrenic Nerve Reconstruction for Diaphragm Paralysis

Phrenic nerve reconstruction has emerged as a safe and effective technique for the surgical treatment of diaphragm paralysis. When performed in properly selected patients with unilateral or bilateral diaphragm paralysis, phrenic nerve reconstruction can restore diaphragm contractility, alleviate respiratory dysfunction, and improve quality of life. Phrenic nerve reconstruction also offers the potential for long-term recovery of respiratory function. Treatment failures still have the option to pursue diaphragm plication, while plication failures would not likely be candidates for phrenic nerve reconstruction, thus functional recovery through phrenic nerve reconstruction may be considered as first-line therapy when feasible.
