Central pancreatectomy is a relatively uncommon operation performed for benign and low malignant potential pancreatic body tumors to optimally preserve pancreatic parenchyma. This operation has declined due concerns of increased postoperative pancreatic leak and fistula relative to non-parenchymal sparing techniques. As imaging technology improves in detecting small pancreatic tumors in the pancreatic mid-body, parenchymal preservation is considered more applicable in the modern era. To add further, the rapid adoption of robotic hepatopancreatobiliary surgery further enables more limited dissection with technical capability of performing fine anastomosis if necessary. In this chapter, we describe indications, preoperative preparation, and surgical techniques of central pancreatectomy using a minimally invasive approach.
