Robotic surgery has grown in popularity across all avenues of surgery in recent years, and complex pancreatic surgery is no exception. The evolution of surgical approach to pancreatic surgery from open to laparoscopic to robotic maintains the same careful surgical principles—we present a typical operative approach to distal pancreatectomy at our institution using the DaVinci robotic system. Studies examining the use of a robotic approach to pancreatic surgery have established this approach as feasible, safe, and non-inferior clinical outcomes compared to open and laparoscopic approaches. Additionally, patients undergoing a robotic approach may benefit from decreased conversion-to-open rates and shorter hospital stays compared to laparoscopic approaches. Despite the upfront investment needed for robotic surgery, cost-effectiveness analysis suggests that robotic distal pancreatectomy has lower overall cost compared to open and laparoscopic approaches. The robotic approach to distal pancreatectomies will continue to prove its benefits to patients and surgeons over time and will be applicable to increasingly complex cases.
