Secondary malignant neoplasms of the pancreas are extremely rare, with the kidney being the most common primary tumor site. Colorectal cancer, melanoma, and sarcoma metastasis to the pancreas have also been reported. Clinical presentations are nonspecific, and metastasis to the pancreas is often found incidentally during radiologic surveillance. Endoscopic ultrasound-fine needle aspiration is a common adjunct to computed tomography and magnetic resonance imaging to aid in the diagnosis. Immunohistochemistry for characteristic markers of the primary site tumors can aid in the diagnosis. There are no standard treatment guidelines for metastatic tumors to the pancreas. Treatment strategy is made on an individual basis for each patient and the primary site tumor, treatment response, and prognosis. There is no consensus for surgical resection of metastatic tumors to the pancreas. Improvement in survival after surgical resection has been demonstrated for selected cases.
