Mirizzi Syndrome

• Mirizzi syndrome (MS) is a rare presentation of chronic cholelithiasis in which there is an obstruction of the common bile duct or common hepatic duct secondary to external compression from gallstones impacted in the gallbladder infundibulum or inflammatory changes (Khokhar et al., Cureus 14:e24375, 2022). • When MS is long-standing, natural progression of the disease can lead to external obstruction, erosion, fibrosis, inflammation, ulceration, and formation of cholecystobiliary/cholecystoenteric fistulas (Khokhar et al., Cureus 14:e24375, 2022). • MS most commonly presents with signs and symptoms of chronic or acute cholecystitis with abdominal pain and obstructive jaundice being the most common symptoms. With any patient with obstructive jaundice or ascending cholangitis, it is important to obtain a thorough history and evaluation for cause with MS being added to the differential (Khokhar et al., Cureus 14:e24375, 2022; Jones and Troy, Mirizzi syndrome, StatPearls Publishing, 2023). • MS is a challenge to diagnosis preoperatively, but preoperative diagnosis is imperative to avoid complications.
