If you have made it to this point in the book, I congratulate you. Surgery is a difficult chosen profession, and finding the time to study amid all your other tasks in life (and not end up burned out and socially isolated with some rather unhealthy coping skills) is a true accomplishment. I firmly believe that we aren’t really taught well enough how to take the 4 years of knowledge from medical school, translate that into practical and efficient clinical algorithms for how to approach surgical patients when we are exhausted or overwhelmed or short staffed, and then to finally refine that into 7–10-min evidence-based focused responses in an oral board-type examination format. A practical revision of our medical education system is beyond the scope of this book, but I agree with what you are thinking, that it shouldn’t take 28 years of schooling and sacrifices to become a surgical attending.
