Laparoscopic adjustable gastric banding (LAGB) is rarely performed since its peak in 2008, yet it is still important to be comfortable in managing patients with LAGB and the potential complications. The most common complications of LAGB are erosion, slippage/prolapse, gastroesophageal reflux, and weight regain/ineffective weight loss. Patients with a history of LAGB that present with complaints should always undergo a thorough history and physical examination. Remember to ask about the patient’s weight loss history including their weight before surgery, lowest weight after surgery, and current weight. Weight recurrence can be an indicator of other complications and is important when planning potential treatment options. Remember to ask about prior imaging studies and endoscopic evaluations. Surgical reversal is always an option (removal of gastric band); surgical revision (re-buckling or placing a new gastric band) should only be considered in patients that have achieved and maintained successful weight loss.
