Community Outreach

Community outreach is thought of as the portion of navigation that occurs prior to someone being diagnosed with cancer, but it also is part of the survivorship care delivery model as patients are transitioned back to their community for post-treatment and long-term survivorship monitoring and care. The needs of the community should be the foundation of outreach and healthcare systems and is required through many government and nongovernmental oversight organizations. A community needs assessment is essential so that healthcare organizations understand and address the needs and assets of the community. Although partnerships are the focus of another chapter, it is important for outreach initiatives to develop collaborative partnerships to reach out and create access to care as well as provide cancer education, screening, and patient support. Navigators play an essential role in this ecosystem and are critical to community outreach efforts, often serving as the bridge to prevention, screening, treatment, and survivorship services.
