This is the original paper introducing the concept of total mesorectal excision which has revolutionized the management of rectal cancer. The authors demonstrated that complete removal of the mesorectum during rectal cancer surgery was associated with minimal risk of local recurrence rate. Subsequent studies have indicated that tailored rather than invariably complete mesorectal excision is preferable for higher tumors, but the principles of this technique have remained the same.
