Magnetic Resonance has become the cornerstone of loco-regional staging of rectal cancer but remains suboptimal in the assessment of earlier stage disease and in its ability to discriminate between benign and malignant polyps. This retrospective series from a unit specifically dedicated to the care of large rectal polyps and early rectal cancers illustrates the practical challenges clinicians and in particular surgeons still face despite advancement in imaging techniques.
