Local excision of rectal cancer is in theory a tempting proposition. In fact, there is not only an increasing number of patients interested in organ preservation but also a substantial proportion of patients with significant comorbidities rendering them unsuitable for radical resection. However, only the most favorable T1 lesions are suitable for local excision with satisfactory oncologic outcomes. High risk T1 lesions T2 lesions are associated with dismal oncologic results.
