This trial illustrates the results of total neoadjuvant therapy for rectal cancer followed by watch-and-wait in case of complete or near-complete clinical response. Patients undergoing radical surgery for tumor regrowth during watch-and -wait had a disease-free survival comparable to patients who underwent radical surgery without ever entering a watch-and-wait program. The results of this trial demonstrate the feasibility of organ preservation in half of the patients receiving total neoadjuvant therapy for rectal cancer.
