Ground Reaction Forces in Parkinson’s Disease OFF and ON States Reflect Changes in Muscle Synergies: A Case Study

Gait is one of the most affected abilities in patients with Parkinson’s Disease (PD). Medication is one of the most commonly used treatments for gait disturbances in PD, but prolonged use leads to a decline in its effectiveness in treating negative symptoms of PD, known as the “wearing-off” phenomenon. Muscle synergy analysis has been insightful in motor control studies, especially human gait. However, specific muscle synergy patterns distinguishing ON and OFF states of PD during gait remain unclear. The present study investigates the differences between the OFF and ON states of PD patient gait, specifically in GRFs and muscle synergy parameters. Here we found that ground reaction forces (GRFs) exhibit distinct changes between OFF and ON states, reflecting muscle synergy activation differences. Our findings contribute to understanding force and muscle synergies during different PD gait states and enable comprehensive investigation of gait pattern changes in OFF and ON states of PD.
