This chapter outlines our current institutional approach to radiotherapy for gynecologic malignancies. For each disease site we review the diagnostic work up and radiation planning guidance including simulation, contouring, treatment planning parameters including dose and fractionation, as well as brachytherapy techniques where appropriate. In addition, the management of potential sequelae of treatment, and follow-up guidance is discussed. In describing our current management, we also highlight areas of clinically acceptable variation in practice. As in most clinical settings, individual variations may influence decision-making and there are aspects of care that are guided by experience in the absence of optimal prospective data. The reader must also appreciate that guidance may change due to evolving data.
