Temporal Bone Tumors

Temporal bone presents unique challenges in the treatment of its tumors due to the complex vascular, neural, and sensory structures it contains. Physical examination, imaging, and biopsy, when necessary, are crucial for accurate diagnosis and successful treatment. Advanced imaging techniques aid in surgical planning and differentiation between benign and malignant tumors, while angiography and embolization also aid in the resection of vascular tumors. Computed tomography (CT) is especially useful in determining bone changes, and magnetic resonance imaging (MRI) is useful in evaluating structures such as the brain, meningeal structures, and soft tissue. Among the symptoms of temporal bone tumors are hearing loss, ear pain, balance issues, a sensation of fullness in the ear, weakness or paralysis of facial nerves, and dizziness. Various surgical interventions are available for the treatment of temporal bone tumors, including lateral, posterior, and anterior approaches. Temporal bone resections, including lateral, subtotal, and total resections, are planned by taking into consideration factors such as tumor size, hearing status, preservation of the facial nerve, cranial nerve deficit, and the need for neck dissection. Endoscopic techniques offer minimally invasive options in selected cases. Complications of surgery include vascular bleeding, intracranial problems, CSF leaks, and cranial nerve deficits, which require careful evaluation. Radiosurgery may be the primary treatment for benign tumors or may complement surgery for residual masses.
