Persistent Postural Perceptual Dizziness

Persistent postural perceptual dizziness (PPPD) is a chronic functional disorder of the nervous system characterized by persistent dizziness, subjective vertigo, and a sense of imbalance. PPPD was defined by the Bárány Society in 2017 and includes features of phobic postural vertigo and chronic subjective dizziness. Patients experience non-rotational vertigo that is exacerbated by upright posture, movement, and complex visuals. Anxiety and psychological factors may contribute. The diagnosis of PPPD is based on the patient’s history, as specific criteria must be met. The exact cause of PPPD remains unclear, but recent research suggests that abnormal interactions between different regions of the brain may be involved. Treatment for PPPD includes a variety of approaches, including patient education, medications, vestibular rehabilitation exercises, and physical therapy. PPPD can have a significant impact on social and occupational life. Dizziness and unsteadiness can make it difficult to work, socialize, and participate in daily activities. Anxiety and depression may further reduce quality of life.
