External Fixation in Treatment of a Patient with Periprosthetic Femur Mal-union

Treatment of periprosthetic fractures is a difficult task in the orthopedic surgery (Abdel et al. Int Orthop 39:2005–10, 2015; Wang et al. Clin Biomech 2018, https://doi.org/10.1016/j.clinbiomech.2018.12.001). It is complicated when there are mal- and nonunions with poor bone quality and concomitant HIV infection (Manzotti et al. Acta Biomed Atenei Parmensis 92:e2021296, 2021; Falakassa et al. Iowa Orthop J 34:102–6, 2014). A 46-year-old man was admitted for treatment with a periprosthetic fracture of the right femur, which occurred 2.5 months after the plating the diaphyseal fracture of the same femur. To fix the proximal bone fragment with the stem of the endoprosthesis in the medullary cavity, extracortical clamp devices (SUV-clamps) were used. The plate was not removed and the distal fragment was fixed with threaded pins inserted into the holes of the plate. Gradual deformity correction was performed using an orthopedic hexapod Ortho-SUV Frame (OSF). The deformity correction period was 38 days, and the fixation period 111 days. The result achieved is excellent.
