Ilizarov Correction Ankle Equinus due to Neuropathy

Long-standing traumatic sequelaeTraumatic sequelae, such as lower extremity deformitiesLower extremity deformity, present significant treatment challenges. Soft tissue management often dictates treatment course. We present a patient who had normal bony anatomy but functional disability due to soft tissue contracture, residual to bilateral lower extremity trauma. This 51 year old female had a history of left lower extremity compartment syndrome with resulting left ankle equinovarus contracture. She was treated with a fine wire, adjustable strut Ilizarov external fixator with gradual correction, with the patient participating by lengthening or shortening the telescopic rods herself. Once the basic deformity was corrected, the position was preserved with tendo-Achilles lengthening procedure and casting. The patient achieved a plantigrade foot for improved ambulation. A surprising but welcome result was that active dorsiflexion was restored.
