Step by Step Approach to Cavus Foot Deformity

This is a case of a 14 year old patient with Charcot-Marie-Tooth disease who presented with severe cavovarus deformities of both feet, causing increasing pain, ankle sprains, and difficulty wearing shoes. He underwent radical plantar release, osteotomy of the first metatarsal, Dwyer calcaneal osteotomy, transfer of the tibialis tendon through the interosseous membrane to the middorsum of the foot, and tenodesis of the peroneus longus to brevis. At 2-year follow-up, he continues to wear an articulated AFO but is asymptomatic, with a functional, plantigrade, and painless foot.
