Pulmonary embolism (PE) is a complex disease state with varying degrees of patient presentation and severity of illness. Given the significant morbidity and mortality associated with PE, the assessment and management of patients must be accurate and performed promptly. A thorough understanding and application of pretest probability scoring, diagnostic workup, risk stratification, and treatment options allow for safe and effective care of patients with PE. The pharmacist clinician is a crucial member of the interdisciplinary PE treatment team that guides treatment decisions, with unique expertise in high-risk medications including anticoagulation and thrombolytics. Pharmacist clinicians contribute significantly to the individual and institutional management of PE through their involvement in treatment pathway, safe medication use process, and PE response team development.
