Healthcare Professional Perspectives on Managing Medications in Older People in Different Healthcare Settings
摘要
Managing medications for older adults can be challenging for healthcare professionals (HCPs), due to complexities such as polypharmacy, multimorbidity, and frailty. This chapter examines the perspectives of HCPs on managing medications for older adults, across diverse healthcare settings. This chapter will examine the barriers and enablers of primary care physicians, medical and specialist physicians, pharmacists, nurses, and other HCPs that are involved with managing medications for older adults in primary care or ambulatory care settings, hospital, and residential aged care settings. This chapter concludes by highlighting the multilevel complexities encountered by HCPs and identifies strategies that may be used to overcome these barriers.
