Surgical Approach in Cardiovascular Involvement of Behçet’s Disease

Behçet’s disease (BD) is a vasculitis in which vascular involvement accompanied by venous thrombosis and arterial aneurysm is frequently observed. It is reported that 7–49% of patients with BD have vascular disease. Superficial and/or deep vein thrombosis was detected in 16.4% of our clinical observations. Venous thrombosis develops more frequently than arterial thrombosis. Arterial system pathologies, which are an important cause of morbidity and mortality in BD, mostly occur in the form of pseudoaneurysms in which the three layers of the arterial system are absent. In some patients with BD, the leading diagnostic symptom is vascular involvement. Acute venous thrombosis in BD is treated mainly with immunosuppressives, although the use of anticoagulants is controversial. While the most dangerous form of vascular disease in patients with BD is pulmonary artery aneurysm, aneurysms are frequently seen in the femoral and popliteal arteries in the peripheral arterial system. In addition to immunosuppressive treatment, open surgery or endovascular treatments can also be applied in arterial aneurysms. The aim of this article is to evaluate cardiovascular involvement and surgical treatment methods in BD. The topic was analyzed and discussed by reviewing our clinical experience and current literature.
