Transformer Failure Modes and Effects Analysis (FMEA)

This chapter introduces the concept of Failure Modes and Effects Analysis (FMEA) as an essential part of any serious investigation of transformer or reactor condition, based on the principles of Reliability Centred Maintenance (RCM). RCM is a concept of maintenance planning and action scheduling based on user’s requirements for a safe and proper operation of a given asset. Successful implementation of a RCM program leads to cost effectiveness, greater asset availability and reliability, and a better understanding of the level of risk entailed by the continual operation of the asset.
