Soft tissue sarcomas are rare malignancies with morphological heterogeneity. Different morphology, pattern and grade can be seen in different areas of the same tumor. This may lead to diagnostic differences between trucut and excision material. These errors can be minimized if the person performing the biopsy takes samples from different areas of the tumor and if the pathologist has experience in soft tissue tumors. The orientation marking of the excision material should be made meticulously and sent for pathological examination together with detailed clinical information. As with many malignant tumors, the use of neoadjuvant therapy has become widespread in sarcomas and secondary changes due to therapy such as necrosis, inflammation, degenerative changes, and bizarre-looking cell morphology may be observed. Since soft tissue sarcomas contain many different subtypes and are very rare tumors, they should be handled by an experienced team to minimize errors.
