Undifferentiated pleomorphic sarcoma (UPS), formerly termed malignant fibrous histiocytoma, is a locally aggressive soft tissue malignancy with high rates of local recurrence and metastases. UPS most commonly presents in the extremities but can present elsewhere, including the trunk and retroperitoneum. This case details the surgical management of a 59-year-old man who presented with a recurrent left flank mass, previously treated with wide resection and brachytherapy. Treatment management was complicated by the tumor’s proximity to the lungs and its recurrent nature.
