The Concept of Surgical Margins from a Surgical Perspective

Planning and executing surgery to achieve adequate surgical margins in soft tissue sarcomas enables the surgeon to improve local control. Thus, the understanding of various factors that impact surgical margins is critical in helping to ensure adequate oncologic resection. The sarcoma community is yet to reach a consensus on the consistency of reporting margins after surgical resection in soft tissue sarcomas. As an alternative to using a dichotomous terminology in which the presence or absence of tumor cells at the margin of the excised tissue specimen indicates a positive or negative margin, this chapter discusses the various nuances that are now prevalent in planning and reporting margins.
