Planning primary total hip arthroplasty (THA) is critical to restore the hip’s native biomechanics. Preoperative X-ray is required to determine the correct positioning of implant components, aiming to equalize limb length and restore the hip’s center of rotation. The chapter outlines the steps for radiographic evaluation, including standardizing the patient’s position and using appropriate templating techniques to predict implant sizes and positions. It also discusses the shift from traditional radiographic templating to digital and 3D software for more precise planning. Minimizing leg length discrepancies and ensuring proper implant fit are the most important aspects to improve surgical outcomes and patient satisfaction.
