This research addresses the challenges of conventional Medical Certificate for Cause of Death (MCCD) forms within the Indian healthcare system. Despite historical efforts to establish uniform registration systems, MCCD documentation still faces issues of transparency and accuracy. Drawing upon a human-centered design approach, this study proposes innovative solutions to enhance MCCD forms usability and effectiveness. Key findings reveal significant gaps between reported deaths and medically certified causes, highlighting the need for improved documentation processes. Through iterative design informed by stakeholder feedback, the research introduces a novel framework prioritizing clarity, simplicity, and user-friendliness in MCCD forms. Case studies and pilot implementations demonstrate the feasibility and potential impact of these redesigned forms on healthcare outcomes. This research underscores the transformative potential of rethinking MCCD forms as tools for transparent, empathetic, and collaborative mortality documentation. By leveraging user-centered design principles, the study aims to empower healthcare professionals and foster trust among stakeholders in the Indian healthcare context.
