Topography of Meningioma Recurrences: Localized Versus Diffuse

Despite their often slow-growth and histologically benign nature, a variable percentage of meningiomas exhibits tendency to recur, even after apparently macroscopic complete resection. While demographics, neuroradiological, pathological, surgical, and molecular risk factors of recurrences have largely been studied, the topographic patterns have poorly been investigated in the pertinent literature. This chapter focuses on the spatial clustering patterns of intracranial meningioma recurrences (local-peripheral versus multicentric diffuse). The flat-shape morphology of the tumor and the high Ki67-MIB1 values are the main risk factors for multicentric-diffuse pattern of regrowth. Further studies will investigate the role of the different biomolecular profiles. The management options are also discussed.
