Robotic liver resection for posterosuperior segments offers significant advantages in precision, minimally invasive access, and improved recovery compared to open surgery. The robotic system allows for enhanced visualization through 3D imaging, high-definition magnification, and greater instrument dexterity, which is particularly beneficial when accessing challenging areas such as the posterosuperior liver segments. The robotic platform’s articulated instruments facilitate fine dissection, meticulous vascular control, and accurate parenchymal transection, crucial for minimizing blood loss and reducing complications. Additionally, the ability to perform resection with smaller incisions typically leads to less postoperative pain, quicker recovery, and shorter hospital stays, aligning with the growing emphasis on minimally invasive approaches in hepatic surgery. Despite these advantages, the robotic system presents certain limitations. High operational costs, and a steep learning curve may hinder its widespread adoption. Furthermore, although robotic liver resection has been shown to be safe and effective in experienced hands, the learning curve for complex liver resections remains a barrier. Careful patient selection, adequate learning process and a skilled surgical team are critical for achieving optimal outcomes in robotic liver resections for the posterosuperior segments.
