Diagnostic Perspectives of Borderline Personality Disorder

Borderline personality disorder (BPD) is a challenging diagnosis due to its heterogenous phenotype, its extensively substantial overlap with other psychiatric disorders, and its debated diagnostic validity and reliability and is proposed as a severely chronic psychiatric disorder comprised of heterogenous and psychological and neurobiological vulnerabilities. Based on DSM-5, BPD symptomatology encompasses four main domains: (1) affective and emotional dysregulation; (2) behavioral dysregulation dimensions; (3) interpersonal instability; and (4) cognitive and self-disturbance including paranoid ideation, dissociative symptoms, and identity impairments. In addition, categorical viewpoints present various mental issues as separate types that comprise an expansive heterogenous group of individuals with clear phenotypical symptoms, whereas dimensional perspectives address that mental disorders are extreme variants of one or more psychological attributes contributing to a diagnosis, considering each person on a spectrum ranging from normal to abnormal behaviors. Furthermore, the alternative DSM-5 model for personality disorders Alternative Model of Personality Diosrder (APA 2022) was presented to consider the scientific issues associated with personality disorder classifications and provide a more comprehensive understanding of the extant studies on personality psychopathology, consisting of BPD. Finally, the Hierarchical Taxonomy of Psychopathology (HiTOP) is an experimentally originated, dimensional model of the core dimensions of mental health and psychological symptoms and functioning that comprises mood, anxiety, psychotic, substance use, personality, and other psychiatric disorders. This model proposes an alternative research paradigm which has various relations to categorical rubrics and provides a constellation of empirically derived hierarchal traits that are shared across continuums of psychiatric conditions. According to the HiTOP model, the BPD conceptualization integrates traits of the internalizing spectrum, certain to the subfactor of Distress, comprising Anxiousness, Emotional Lability, Hostility, Perseveration, low Restricted affectivity, Separation Insecurity, Submissiveness, Identity problems, Negative relationships, Fragility, Ineptitude, low Invulnerability, Antagonistic Externalizing spectrum such as Rudeness, Flirtatiousness, and low Timorousness.
