Crooked noses affecting the dorsal segment, caudal segment, and the junction between these segments are not rare. The main reasons for these kinds of deformities are trauma and previous surgery. There are three options in the surgical management, namely splinting, resecting and reconstructing, and cutting and overlapping. Spreader grafts, caudal septal extension grafts, and/or L-strut grafts are the main techniques for splinting the L-strut framework. Subtotal septal reconstruction and extracorporeal septoplasty are more challenging techniques for the surgeon, and they should be reserved for extremely crooked cases. In rare occasions, cutting and overlapping can also be used when the L-strut framework is bigger than normal.
