Lower limb nerve injuries can be devastating; particularly, injury to the common peroneal nerve (CPN) can be significantly detrimental as it can lead to foot drop or loss in ankle dorsiflexion. CPN injury is accompanied by loss of eversion and extension of the toes and often presents with sensory deficits in the lower leg and dorsum of the foot. Primary treatment will involve nerve repair or nerve transfers whenever possible. The management of CPN injuries that do not recover is challenging. When a patient has CPN palsy without signs of recovery, tendon transfer is the typical secondary procedure to restore some degree of ankle dorsiflexion. Anterior transfer of the posterior tibialis tendon (PTT) is the preferred operative treatment of chronic footdrop (Stevoska et al., 143:773–784, 2023).
