In severe brachial plexus injuries where four or five roots (spinal nerves) are avulsed, there may be insufficient donor axons on the injured side. In this case, a contralateral C7 transfer may be useful, and the use of a retropharyngeal tunnel maximizes the nerve length and obviates the need for long grafts.
