Injuries to the brachial plexus and upper extremity nerves are fraught with devastating dysfunction and disability. Proximal nerve injuries are more problematic, as the time needed for distal nerve regeneration likely exceeds the lifespan of the distal motor end plates. Thus, nerve transfers involve coapting an expendable donor nerve to a recipient nerve in order to restore specific end organ function. In this chapter, we will discuss the utility of median nerve motor branch to posterior interosseous nerve transfers to restore wrist and finger extension.
