The dorsal scapular nerve originates from the posterior aspect of C4 and C5 and occasionally also from C6. It perforates the scalenus medius to descend between it and the scalenus posterior in the space known as the minor interscalene triangle. At this point, it joins with the deep branch of the transverse cervical artery (also known as dorsal scapular artery) and travels toward the medial border of the scapula. It reaches the levator scapulae and the minor and major rhomboideus muscles, which it innervates.
