Solid Organ Transplant Prehabilitation

Prehabilitation has emerged as a critical strategy in optimizing outcomes for solid organ transplant candidates. This chapter explores the current landscape of prehabilitation in liver, lung, kidney, and heart transplantation, focusing on its potential to mitigate frailty and improve both pre- and post-transplant outcomes. The chapter begins by examining the prevalence and impact of frailty in transplant candidates, highlighting its association with poor outcomes independent of disease severity. The chapter then reviews validated functional assessment tools. The relationship between organ dysfunction and contributors to frailty such as sarcopenia and poor nutrition is described. The core of the chapter is devoted to evidence-based prehabilitation interventions. We analyze the efficacy of various exercise modalities, including aerobic and resistance training, in improving functional capacity and quality of life across different organ groups within the context of typical comorbidities of end stage organ disease. Nutritional interventions, particularly those targeting BMI normalization and protein intake, are discussed in the context of their potential to enhance transplant candidacy and post-operative recovery. Additionally, we explore the often-overlooked psychosocial aspects of prehabilitation, presenting emerging data on interventions addressing anxiety, depression, and overall mental well-being in transplant candidates. The chapter concludes by identifying key areas for future research, including the need for standardized prehabilitation protocols and long-term outcome studies. We argue that a multimodal approach to prehabilitation, addressing physical, nutritional, and psychosocial factors, offers the greatest potential to optimize transplant candidates’ health and improve overall transplant success rates.
