Ureteropelvic junction obstruction (UPJO) is one of the most common causes of prenatal hydronephrosis and may require surgical repair. Robotic pyeloplasty is the most commonly performed pediatric robotic surgery. Robotic pyeloplasty offers advantages over open pyeloplasty including less pain, shorter length of stay, quicker recovery, smaller incisions, and improved cosmesis. Pediatric urologists have continued to broaden the use of pediatric robotic pyeloplasty to include younger and smaller patients. In this chapter, we describe transperitoneal robotic pyeloplasty in pediatric patients using a traditional approach, a Hidden Incisions Endoscopic Surgery approach, a transmesenteric approach, and a retrocolic approach.
