Robotic surgery has fundamentally changed the management of prostate cancer. Over the last two decades, we have seen several new robotic approaches developed in the quest to continue to improve outcomes of patients undergoing radical prostatectomy. While open radical prostatectomy was performed primarily using the retropubic approach, robotic prostatectomy is performed via the extraperitoneal, transperitoneal, and perineal routes. There are benefits and drawbacks to each approach. In this chapter we describe our experience with the extraperitoneal technique, which has been our preferred approach to further decrease surgical invasiveness, using either a single port or multiport robot.
