In the standard radical prostatectomy approach, the dorsal vein complex and puboprostatic ligaments are divided and endopelvic fascia on either side of the prostate is incised to free prostate from its surrounding attachments. Retzius sparing/pelvic fascia sparing robot-assisted radical prostatectomy (P-RARP) approach was first described by Galfano et al. in 2010. A step-by-step description of the P-RARP surgical technique is provided to preserve pelvic fascial structures thought to play role in urinary continence. P-RARP showed significant advantage over standard RARP in terms of immediate urinary continence recovery in comparative studies. Further research is needed to compare cancer control and health-related quality of life following P-RARP and standard RARP.
