The technique of robot-assisted urinary diversion has evolved significantly since its inception more than 10 years ago. Several high-volume centers have reported standardized techniques with refinements and subsequent outcomes (Desai MM et al., J Urol 198:436–444, 2017). In centers of excellence, up to 97% of diversions are now performed intracorporeally (Hussein AA et al., J Urol 199:1302–1311, 2018), including complex bladder substitutes (Wilson TG et al., Eur Urol 67:363–375, 2015; Guru K et al., Urology 76:866–871, 2010; Collins JW et al., Robot-assisted intracorporeal neobladder: the Karolinska standardized technique. In: Wiklund P, Mottrie A, Gundeti MS, Patel V, editors. Robotic urologic surgery. Cham: Springer International Publishing; 2022. p. 673–9). While performing robot-assisted urinary diversion, three key phases exist: first, preoperative physical and psychological preparation of the patient. Second, intraoperative safe execution of the procedure; and third, regular and prompt assessment and intervention for any complications postoperatively. The aim of this chapter is to outline patient preparation and positioning, theatre and trocar setup, special equipment required, and essential surgical steps when performing robot-assisted ileal conduit, orthotopic bladder substitution, or cutaneous ureterostomy.
