The ideal assessment process in the Primary Care Behavioral Health (PCBH) model is one that promotes a contextual understanding of the issue or problem that the patient wants help with and engages the patient in the belief that behavior change will be useful. How does the problem or issue influence their functioning in life? When did it start? What are their resources for addressing the problem? Does the problem result in barriers to their living the life they want? These are some of the questions that inform PCBH assessment, and it is the patient’s answers that lead to the development of an action plan to experiment, to try something different to improve their quality of life. This chapter introduces several approaches to PCBH assessment and interventions, including Focused Acceptance and Commitment Therapy (FACT) and the 5As model. Throughout the chapter, we reference Appendices D (Patient Education Handouts), E (FACT “Visit Flow” Tools), F (FACT Skill Development Toolkit), and G (Interventions Commonly Used in FACT), which are available online at http://behavioralconsultationandprimarycare.com).
