Pathways are the workflows, simple and complex, that redefine primary care (PC) work and move teams quickly to use of more evidence-based behavior change interventions for the many people who receive no care or care based on little or out-of-date evidence. While they are not easy to create, once created, successful pathways are very durable and quite magical in their impact on PC. It is in creating pathways and embracing the concept of working with more than one patient at a time, and as a member of a team, that we can begin to see a faster and better way forward for healthcare. This chapter provides guidance on pathway development and the start of class-based services in PC, which are a feature of many pathways.
