Drug Allergy in Children

Drug allergies constitute a significant healthcare concern because of their frequency and the entity of their manifestations, ranging from mild skin rashes to life-threatening anaphylaxis. This chapter provides an overview of drug allergy epidemiology, mechanisms, diagnosis, and management strategies. The development of drug allergies involves complex immunological processes. Drugs can act as haptens, binding in various ways to endogenous proteins and triggering an immune response that involves the activation of several immune cells, resulting in the release of inflammatory mediators such as histamine. Genetic predisposition and prior sensitization play pivotal roles in determining individual susceptibility to drug allergy. Accurate diagnosis is therefore essential to evaluate drug discontinuation. Patient history, combined with diagnostic tools such as skin tests, in vitro tests, and drug provocation tests, represents the crucial step of the allergological workup. However, the diagnostic tools may have limitations, necessitating careful interpretation by experienced clinicians. When identified, the cornerstone of drug allergy management lies in the avoidance of the implicated drug and the use of alternative medications. When that is not possible, drug desensitization protocols have proved to be an effective strategy. In conclusion, drug allergies are intricate immune responses that can lead to various clinical manifestations. Understanding the underlying mechanisms, precise diagnosis, and effective management are essential for providing optimal patient care. Collaborative efforts between clinicians, researchers, and patients are needed to address this challenge and to enhance our ability to prevent and manage drug hypersensitivity reactions effectively.
