Relative Femoral Neck Lengthening with Advancement of the Greater Trochanter

A high or malpositioned greater trochanter may be responsible for mechanical and muscular dysfunction of the hip. Both extra- and intra-articular impingement may also be associated with decreased muscle function due to a short abductor lever arm or shortened muscle action length. In comparable situations it is may be necessary to advance the greater trochanter. In the native hip, care has to be taken not to damage the blood supply of the femoral head. For correct and safe treatment, the technique of relative neck lengthening is recommended. Technically, there are similarities with the development of a retinacular flap.
