Clinical Assessment of Hip Dysfunction

Patient history and the physical exam including supine and prone range of motion, strength and provocative tests each play an important role in the assessment of hip dysfunction. As femoroacetabular impingement is a diagnosis that is based on historical and clinical aspects and not just on radiological findings, emphasis on its recognition must be placed on the patient’s anamnesis, complaints, and description of pain. A detailed physical exam further suggests the diagnosis of the particular hip dysfunction. Observation of undressing and inspection can be used to identify quality-of-life limitations. Analysis of limping can indicate muscle weakness and provide information on femoral version and gait compensation. Results of range-of-motion tests may indicate a variety of hip dysfunctions including dysplasia and impingement. Provocative and strength tests help to differentiate and pinpoint the morphological correlate of symptoms.
