The Arc of Conversation: Foundations of the Technique

This chapter discusses the foundations of the Arc of Conversation technique, which is based on the premise that an investigation of patient and family experience of illness and understanding of a diagnosis must occur before providing education about disease. The chapter begins by explaining the clinician’s role as a guide in helping patients and families navigate the medical journey with empathy, patience, and expertise. The author shares a personal anecdote of how she developed a conversational approach to the initial consultation with clients as a wedding photographer. This approach, which focused on understanding the couple’s vision and priorities and addressing their fears and concerns, provided a valuable framework for developing the Arc of Conversation technique in palliative care. The chapter introduces the foundation of the technique: to investigate experience and understanding before providing education. It introduces the four arcs of focus in the technique: Illness Arc, Disease Arc, End-of-Life Wishes Arc, and Hospice Care Arc. By following the foundational technique of investigation before education, healthcare professionals can ensure that they engage in effective and informative conversations with patients and families regarding any new topic.
