Asymptomatic Hypertension in Pregnancy

Hypertension diagnosed in a pregnant patient for whom there are no concomitant symptoms can indicate chronic hypertension or gestational hypertension. Either way, feared complications of hypertension during pregnancy can include preeclampsia or Hemolysis, Elevated Liver Enzymes, and Low Platelets (HELLP) syndrome. It is not common for a pregnant patient to seek care in the emergency setting for abnormal blood pressure values. Unlike a general adult with hypertension, it is important to evaluate for end-organ injury and to evaluate for HELLP syndrome in every asymptomatic patient with hypertension and initiate interventions as necessary. Gestational hypertension is diagnosed after 20 weeks of gestation and typically requires pharmaceutical intervention. Any complications of hypertension during pregnancy will typically require more concerted interventions in collaboration with a specialist.
