Multiple sclerosis (MS) is a prevalent chronic disorder characterized by inflammation and demyelination in the central nervous system (CNS), leading to varying degrees of axonal and neuronal damage. It affects approximately 2.8 million people worldwide, predominantly young adults. The underlying cause involves a breakdown in immune tolerance, although no single autoantigen or autoantibody has been definitively linked to MS. The disease manifests primarily in two forms: relapsing-remitting MS (RRMS) and progressive MS (PMS), with the latter marked by a gradual accumulation of disability. Diagnostic criteria for MS have evolved over time to incorporate advances in understanding and diagnostic techniques. Historical criteria, dating back to the nineteenth century, initially focused on clinical symptoms but later emphasized the importance of imaging findings, such as dissemination in space and time. The McDonald criteria, introduced in 2001 and subsequently revised, streamlined the diagnostic process by integrating clinical and MRI evidence. These criteria prioritize objective demonstration of CNS lesions and dissemination in space and time, crucial for accurate diagnosis. However, distinguishing MS from other conditions that mimic its symptoms remains essential. Disorders like neuromyelitis optica spectrum disorder (NMOSD) and myelin oligodendrocyte glycoprotein antibody–associated disease (MOGAD) can present similarly but require distinct management approaches. While there is no cure for MS, various disease-modifying therapies (DMTs) aim to reduce relapses and slow disability progression, with early treatment showing promise in limiting long-term disability. Moreover, lifestyle factors such as sunlight exposure, vitamin D levels, and smoking cessation may influence MS risk. In conclusion, understanding the complexities of MS diagnosis and treatment is critical in improving outcomes and quality of life for individuals affected by this debilitating condition.
